Barbara Pisano Messing is a practicing speech pathologist with 36 years of clinical and administrative experience. Dr. Messing was the administrative-clinical director of the Milton J. Dance, Jr. Head and Neck Center, Head and Neck Surgery, and Johns Hopkins Voice Center at GBMC in Baltimore, Maryland, for 22 years. She is recognized as a clinical specialist in head and neck rehabilitation and a board-certified specialist in swallowing and swallowing disorders. She has published many peer-reviewed manuscripts and several book chapters. She lectures nationally and internationally on dysphagia, esophageal disorders, and clinical pathways and outcomes in head and neck cancer. Her clinical and research interests are in the area of head and neck cancer rehabilitation, dysphagia, and voice disorders. She received her doctorate at the University of Queensland, focusing her research on dysphagia in head and neck cancer patients. She completed her PhD program at the University of Queensland in 2020, and she received the Dean's Award for Outstanding Higher Degree by Research Theses for her thesis, entitled An Examination of Functional Outcomes Following Head and Neck Cancer and Enhancing New Pathways of Care.

Head and Neck Cancer: Implications of Surgery and Reconstruction
This course seeks to fill a knowledge gap in the advanced instruction of head and neck cancer (HNC) management. This lecture will identify current surgical treatment options for HNC patients to improve early and long-term outcomes and overall survival. Specifically, this course provides instruction on the impact of surgery and reconstruction on communication and swallowing in head and neck cancer patients post surgery. This discussion includes the anatomy and physiology of HNC, as well as tumor classification, cancer staging, and factors influencing reconstruction options in HNC. This course includes an interview with a head and neck surgeon, Dr. Farzad Masroor, who discusses reconstruction options, the decision-making process on flap selection based on tumor location, the extent of surgery, patient factors, the impact on swallowing, and survival outcomes.

Multidisciplinary Head and Neck Cancer Clinical Pathway Implementation
This course seeks to fill a knowledge gap of advanced instruction in head and neck cancer (HNC) management. Speech pathologists with an interest in obtaining advanced-level competencies in head and neck cancer are provided with best practice service delivery models appropriate for the management of HNC patients before, during, and after treatment. Specifically, the course will describe the purpose and implementation of a multidisciplinary head and neck clinical pathway designed to optimize treatment and functional head and neck cancer outcomes. Current national and international models of HNC care delivery designed to manage patients with complex needs are addressed. The course will discuss evidence-based literature to guide speech pathologists in providing exercise protocols and identifying clinician- and patient-related outcome measures at specified time points to promptly identify and address functional deficits in a proactive approach over time.

Participants will learn to identify potential issues during the diagnostic workup for cancer staging and treatment decision-making. A brief overview of HNC risk factors, initial diagnostic processes (i.e., staging, etc.), indications for surgical and nonsurgical treatment, and effects of treatment on functional outcomes will be discussed. This course is appropriate for speech pathologists, oncology nurses, physical and occupational therapists, otolaryngologists, and medical residents working in any setting but particularly those in graduate training programs, acute care hospitals, cancer centers, and rehabilitation programs/centers.

This course gives students an overview of esophageal disorders and the implications for speech pathologists in the diagnosis and management of esophageal dysphagia. A brief review of specific esophageal abnormalities commonly impacting dysphagia is presented. Participants will learn to identify the significance of the interrelationships between oral, pharyngeal, and esophageal disorders on dysphagia diagnosis and management. Instrumental evaluation tools and protocols will be addressed. This course is appropriate for speech pathologists responsible for the management of patients with swallowing disorders in any setting, but particularly targets those in acute medical and rehabilitation settings.
