Mike Studer

PT, DPT, MHS, NCS, CEEAA, CWT, CSST, CBFP, CSRP, FAPTA

Mike Studer received his physical therapy degree from the University of Missouri-Columbia in 1991. He received his postprofessional MHS degree in physical therapy with neurologic emphasis from the University of Indianapolis. He has been board-certified as a clinical specialist in neurologic physical therapy since 1995 and designated a certified exercise expert in the aging adult (CEEAA) by the Academy of Geriatric Physical Therapy since 2011. Mike has served as the vice president of the Academy of Neurologic Physical Therapy of the American Physical Therapy Association (APTA) and has been the chair and vice-chair of several special interest groups at the national level in each of the Academies of Neurologic and Geriatric Physical Therapy, including balance and falls, stroke, and the practice committee in neurology. He is a full-time treating therapist and founder of Northwest Rehabilitation Associates in Oregon. Dr. Studer has presented courses and published articles on neurologic and geriatric rehabilitation since 1995 and has authored and coauthored more than 30 articles on topics of neurology and geriatrics, as well as several book chapters on stroke, cognition, Parkinson's disease, and preventive care. Mike is now regularly a guest lecturer on a national and international basis at several universities and national meetings on the topics of stroke, Parkinson's disease, balance, motor control, motor learning, cognitive impairment, and case management. He was awarded the 2011 Clinician of the Year award by the Academy of Neurologic Physical Therapy, and in 2014, he received the same award from the Academy of Geriatric Physical Therapy. He was the first to receive this national distinction from each entity.

In 1933, President Franklin D. Roosevelt said, "The only thing we have to fear is fear itself." Fear is the most pervasive barrier inhibiting recovery and function across the life span. Fear-avoidant behavior is well established as a reason not to move. However, fear also impacts how we move (symmetry, speed, compensations), offering opportunities for intervention. Changes due to fear include reduced step length, foot clearance, and gait speed for persons with fear of falling; urgency in those with incontinence; limited head motion for those with fear of dizziness; gait deviations, word-finding difficulties, and altered UE biomechanics after stroke; depression in MCI; and movement substitutions to retrieve something from the floor or arise to standing for those in fear of pain. Functionally, we see the consequence of reduced participation (movement, social, vocational) as an unfortunate and unnecessary commonality of all. The time has come for PTs, OTs, and SLPs to recognize that we must directly treat fear itself, not just as a consequence of physical impairments (imbalance, weakness, pain, tone, dizziness) or cognitive impairments (word-finding, reduced working memory). We must come together in addressing fear directly, accomplishing this by systematically addressing the common threads of maladaptive neuroplasticity (sensory, motor, psychological) that lead to altered motor control and limited participation emanating from fear.

Disease modification, compensation, and rehabilitation for Parkinson's disease (PD) are three different tracts. Within these, it is common to see the rehabilitation for Parkinson's disease discussed as "one approach" and oversimplified into a one-size-fits-all schema. This course will further delineate the subtypes of PD, include a thorough discussion of the various subtypes (phenotypes), and begin to ascribe treatment strategies specific to each presentation. We will address the primary symptoms in PD. Thereafter, the course builds on this well-established classification paradigm for patients with primary tremor and rigidity and those with primary dyskinesia, freezing of gait, posture, and festination. Clinicians in the audience will be invited to use the paradigm to further the discussion of subtyping PD and researching within these categories as well as extend into matters of compensation and disease modification. We will provide video and case study application across three different classifications of Parkinson's disease and reveal applicable treatment strategies that are salient for all rehabilitation professionals in the acute, home, inpatient (SNF and IRF), and outpatient settings.
