Tracey Collins

PT, PhD, MBA, GCS

Tracey Collins lives in Clarks Summit, PA, but is originally from Endicott, NY, and is currently an assistant professor at the University of Scranton. She is a board-certified specialist in geriatrics. She has practiced in home health and hospice for more than 20 years. She continues to practice in home health through Commonwealth Home Health and VNA of Lackawanna County. She received her BS in PT from the University of Scranton in 1989, her MBA from UNC-Wilmington in 1997, and her PhD in Administration from Walden University in 2006. She has 17 scholarly publications and has presented 27 times at national physical therapy conferences.

Dr. Collins is the current research chair for the Home Health Section of the APTA and has served on the Home Health Section research and practice committees and the Academy of Geriatrics finance committee. She lived in North Carolina for nine years and was the treasurer for the NCPTA from 1997-2000. She enjoys spending time with her three children and returning to Wilmington, NC, for vacations.

Patient-centered care is the cornerstone of patient-centered medical homes (PCMHs) and affordable care organizations (ACOs) as presented in the Affordable Care Act (ACA). It also is essential in meeting the Triple Aim of the Institute for Healthcare Improvement (IHI): better care for individuals, better health for populations, and lower per capita costs. Unfortunately, many health care providers have not been adequately trained in motivational interviewing or health coaching as part of patient-centered care. This course will provide detailed background information on motivational interviewing and health coaching as well as examples of interview questions and techniques to use with your patients to help move toward patient-centered care.
